FASD Clinic Social Worker Contract Position
Full-time – Contract until December 31st, 2025, with potential for extension.

The Program – Neurodevelopmental Assessment & Diagnostic Centre (NDC)
Enviros’ Fetal Alcohol Spectrum Disorder (FASD) Diagnostic and Assessment Clinic for both children and adults.
The Position – Social Worker
The Social Worker will assist, along with the Clinical Team, in providing recommendations and resources to individuals who have been recently diagnosed with FASD to better understand their unique strengths and provide individualized referrals to support some of their complex needs. The Social Worker will liaise and connect with post-assessment services and resources that best serve the client. The Social Worker, along with the lead Coordinator and Manager, will promote strong community partnerships; including collaboration with the Justice sector. The Social Worker will facilitate Wrap-Around style meetings to bring other service providers together to better serve the client and ensure there’s a cohesive plan to help support the client (or caregiver) reach the goals they have identified.
Duties will include:
· Supporting caregivers and/or adults as they enter the diagnostic process and then when transitioning to post-assessment supports
· Breakdown of resources and supports based on results from assessment report
· Advocacy and collaboration throughout the sector
· Liaising and transitional support for individuals on post-assessment program waitlists
· Ensuring referrals are made to programs identified in the recommendation portion of the assessment report
· Work with clients and/or families to ensure all supporting documentation is completed
· Supports and interacts with individuals and/or families in a way that incorporates their needs, perspective, preference and experience
· Educates individuals and /or families on the diagnostic process and what to expect at each appointment; assist the individuals and/or families in getting to each appointment
· Facilitate completion of standardized questionnaires
· Educate social service providers in the referral and assessment process for FASD
· Communicate best practice strategies based on assessment results
· Collaborate with community and intersectoral groups and assist with community capacity building for pre and post assessment education and support
· Compile reports and collect data as required for outcomes and evaluation purposes
· Assist physician with filling out disability forms etc
· Maintain program training standards as required
Qualifications:
· Minimum degree in social work or other related social services field is required
· Ability to create/lead and maintain effective team relationships
· Ability to work both independently and within a multi-disciplinary team environment
· Strong organizational skills
· Experience working with teams of various professional backgrounds
· Minimum of 3 years experience working in a community setting with vulnerable populations.
· Demonstrated ability to work with individuals from diverse backgrounds and cultures using a supportive, non-judgmental approach
· Experience in working from a trauma informed lens
· Demonstrated ability to collaborate effectively with various community partners.
· Excellent written and verbal communication skills
· Knowledge and understanding of FASD
· Computer literacy
· Reliable vehicle
· Clear Criminal and Child Intervention check completed
