About The Triveri House

Triveri House is a youth focused Place Based Supportive Housing program providing a range of housing and support needs for young adults aged 18 to 29 who are at risk of or are currently experiencing chronic and episodic homelessness. The Program works to provide on site intensive case management and participant led goal planning to work towards independence and transition into long-term housing stability within participant’s chosen communities.

The housing needs that young adults face are as individualized as each person. With this in mind, Triveri House provides a range programming to meet these needs. The programming operates under a Place Based Supportive Housing framework and integrates three types of housing models which include:

  1. Longer-term housing for those with more complex needs and the need for a stable home base;
  2. Transitional housing for those who need support, but the level complexity is not as high. The goal will be for these individuals to move within 24 months
  3. Rapid Re-housing for those individuals who may be considered less complex but are finding housing as a barrier to move forward in their life. These individuals will stay in the program approximately 6 to 12 months.

Triveri House aims to employ wrap around supports that meet the individualized needs of participants along their journey of recovery and stabilization through principle-based practice and working alongside participants as the leader of their own care plan as they transition into lesser intensive services and begin their reintegration into sustainable community independence. Triveri House aims to meet participants where they are at within their individual stages of change, and work to develop individualized and targeted supports to promote the wellbeing of the participant’s it serves.

The overarching goals include:

  1. increased connection with natural supports,
  2. increased access to community resources, stable income and food security,
  3. a self-directed exit from the Homeless Serving System of Care

The program will achieve this through providing one room occupancy units at Triveri House along with a staffing model of case managers, support staff and additional supports through the community.
