Young Adult Supports

Youth in the program typically:

  • Are between the ages of 15–22
  • Are transitioning out of foster care or live-in group care
  • Are unable to return to their original home environments
  • Have little or no parental or caregiver support
  • Are seeking guidance and stability as they move toward independence

The Youth Transitions to Adulthood (YTA) program walks alongside youth and young adults ages 15 to 22 as they take their first steps toward independence. Many participants have spent much of their lives involved with Child and Family Services and are entering adulthood without the consistent support of family or a safe place to land. YTA exists to ensure they do not have to navigate this transition alone.

Each young person is thoughtfully matched with a dedicated YTA staff member who builds trust through consistency, care, and genuine connection. For youth who may feel isolated or abandoned, these relationships become a steady source of guidance, encouragement, and belief in their potential.

Through regular one-on-one support, youth learn essential life skills at their own pace—managing money, finding housing or employment, accessing mental health and addiction supports, and learning how to navigate daily responsibilities with confidence. Every plan is youth-led, honoring their voice, goals, and strengths.

YTA supports approximately 24–26 youth at any given time, with most participants involved for about 18 months. Graduation comes when youth feel ready to stand on their own—confident, capable, and hopeful about their future. YTA is not just about learning skills; it’s about building stability, self-worth, and a belief that a different future is possible.

Referrals to the Youth Transitions to Adulthood (YTA) program are made through Children and Family Services and coordinated by a placement coordinator.
